Sally Lake RN, BSN received her Bachelor’s of Science degree in 1979 at Baylor University. She has New Orleans roots and attributes this to her outgoing personality. Prior to joining Dr. Evans practice in 2005, she was a well-respected  in cardiovascular medicine. It was her long-time passion of women’s health issues that ignited her move to Premiere Center for Health & Wellness.

Working closely with Dr. Evans for over 5 years, Sally has gained a true understanding of women’s health issues and is passionate about helping our patients look and feel better through hormone replacement and cosmetic procedures. Along with Dr. Evans, Sally regularly attends workshops throughout the country to enhance her knowledge of hormone health and cosmetic procedures.

Sally’s patients describe her as a fun-loving person who loves to laugh and genuinely cares about her patient’s overall well-being. Her compassionate demeanor is apparent by the care and general understanding she displays with her patients.

 Sally resides in the Landen area with her husband and 4 children, including triplets. She and her husband enjoy spending time attending their children's sports events and spending time together. She loves to read and is self described as enjoying crime stories and forensic science.
