| How to Prepare for your Gynecology Visit--to make it work best for you!! |

Dr. Evans wants your Gynecology visit to go smoothly and take care of any problems you may be having. Consider following the following suggestions to make your visit work for you.
- LET US KNOW IF YOU NEED EXTRA TIME WITH DR EVANS:
Let the office know if you are having a problem and need time to discuss the problem with Dr. Evans. Unfortunately, as insurance companies continue to lower payments, physicians are forced to spend less time with patients. This is OK for patients who are having no problems and simply need a routine check up and/or prescription refills. However, when someone is having problems, Dr. Evans would like to provide the time needed to address the problem(s). If the office does not know to allow for more time, then spending unscheduled time with a patient will make Dr. Evans late seeing other patients, and those patients may become angry that they are inconvenienced and made to wait. So simply let us know that you are having problems to discuss at your visit, and you need extra time with Dr. Evans, and we will schedule your appointment accordingly. Please also understand that when you are having problem(s), and wish to address them at a routine gynecology visit thus requiring a lengthier office visit, most insurance companies require that we charge two separate co-pays, one for the annual exam, and one for the problem visit.
- CREATE A LIST OF YOUR QUESTIONS/CONCERNS.
Bring this list to your visit to review with Dr. Evans. This will ensure that your problems and concerns are addressed during your visit.
- BRING TEST RESULTS WITH YOU.
If you would like Dr. Evans to address the results of a test you have had performed, such as a Pap test or ultrasound report, then please bring a copy of the result with you or make sure it has been sent to AND received by the office. Dr. Evans' staff can request a copy of the report while you are at the office but it is unlikely that the report/result will be sent immediately to the office. So if you don't bring a copy of the report/result with you then you will likely need to make another appointment to review the result/report after it becomes available for Dr. Evans to review.
- If you are a NEW PATIENT and have had an uncomplicated medical history it is generally not necessary or beneficial to bring a copy of your prior record. However, it you have had an unusual or complicated history it is quite beneficial to supply us with a copy of your prior records.
- Please feel free to BRING ANY FEMALE COMPANION to your office visit. However if you would like for Dr. Evans to discuss your evaluation and treatment with a male companion/spouse/significant other then it will be necessary to make arrangements to be seen at a time when other female patients are not in the office.
- COMPLETE YOUR PAPERWORK AHEAD OF TIME AND BRING YOUR INSURANCE CARD TO THE OFFICE.
Dr. Evans has worked extensively to create a secure online option for all patients to be able to complete almost all paperwork at any time before coming to the office. You may do this by choosing the "online patient features" option on this website. Create a patient account--this is a complimentary secure feature and will allow you to complete necessary paperwork--optimally 2-5 days before your office appointment. After you create an account, go to the "health forms" section and fill out the New Patient/or Returning patient form. Complete the menopause assessment form if you are having menopuase symptoms. Look for any other forms that may apply to you and fill them out. These forms will come directly to us online in a completely secure system. If you are unable to take advantage of this option, please come to the office 20-30 minutes before your scheduled appointment so that you can complete your paperwork and be ready to be seen at your appointment time. If you have not completed your paperwork before your scheduled time it creates delays in the office schedule and becomes an imposition for other patients. We understand that many patients become frustrated that paperwork must be completed, however, the medical and demographic information that we gain from the paperwork is required by your insurance company and must be completed before you can be seen.
- BE FAMILIAR WITH YOUR INSURANCE PLAN.
Dr. Evans and staff make efforts to be familiar with the logistic requirements of the most common plans, however even major carriers have multiple plans and it is not possible for the office to know the specifics of each plan. Insurance plans are typically requiring higher deductibles and/or co-pays in attempts to keep insurance premiums lower. Your insurance company is legally required to inform us of your financial obligations under your plan, but we are not always privy to information such as network laboratories or preferred medication options. Therefore, it is in your best interest for you to be familiar with any specific requirement of your plan as it affects your visit, and to relay that information to us.
- Dr. Evans' staff will verify your insurance benefits prior to your arrival at the office, when you provide us with your insurance information at least 48 hours in advance. You will be expected to pay your portion (co-pays and/or deductibles) at the time of service. If you are unable to provide your insurance information ahead of time we will attempt to verify your benefits before you are seen. This may delay your appointment time. If we are unable to verify your benefits you may be asked to provide a deposit that will be reimbursed when your insurance claim has been paid.
- PLEASE RESPECT OUR CANCELLATION POLICY
Dr. Evans and her staff begin preparation for your office visit at least 48 hours in advance. We staff the office accordingly, and do our best to make your appointment pleasant and efficient. Please show respect for our time and efforts by notifying us at least 2 full business days in advance if you will be unable to attend your scheduled appointment. We will collect a deposit from you to hold appointment times after 1 missed appointment for new patients and ultrasound appointments, and after 2 missed appointments for Returning Patient appointments (a missed appoinment includes any appoinment cancelled with less than 1 full business day's notice). The deposit will be applied to your office visit after you are seen, but will not be refunded if the appointment is missed or cancelled with less than 1 full business day's notice. Of course we will consider exceptions to this policy when extenuating circumstances exist. Thank you for your respect and understanding.
