Dr. Evans Office is contracted with most insurance companies, however, you should always check with your insurance company before making an appointment to avoid any confusion and any unexpected out of pocket expense at your time of service.

The insurance plans that we currently participate in are:

• Anthem (except Anthem Senior Advantage and Blue Priority)
• Choice Care Humana (except TriHealth Humana and Humana New Health)
• Health Alliance
• Alliance Partners
• Medical Mutual of Ohio
• United Medical Resources ( Only certain UMR plans)

Our policy with insurance companies that we are NOT contracted with is for the patient to pay the full amount for their visit at or before their time of service.  We will submit a claim to the patient's insurance company (except for Medicare, we will not bill) for them.  If the insurance company pays any out of network benefits to us the patient will be properly reimbursed.
